Patient Responsibilities
Referrals/Coverage -We request that you contact your insurance company prior to your initial visit to determine what your financial and referral/authorization responsibilities are for physical therapy.

Payment
Unfortunately, most insurance plans do not provide full coverage for physical therapy and patients have some financial responsibility for their treatment. We kindly ask for all payments to be made at the time of service. Please determine if your insurance plan has one of the following, and how much you are responsible for:

• Co-Payment
• Deductible
• Co-Insurance

What is needed for your first appointment

• Insurance card
• Prescription from referring doctor (if required by your insurance)
• Verification of co-payment, deductible or co-insurance amount from your insurance company
• Primary care referral (if required by insurance)
